They will verify if the provider is/is not currently contracted with the Evolutions network. If not, SENTRY HEALTH (MAP) can guide members through the process of nominating that provider.
EXPLANATION OF TIERS:
Tier 1 Provider:
- Great news! The provider is participating in the Evolutions Prime Tier 1 Network.
- Provides the best member benefit option resulting in the lowest copay/cost to members and the plan.
Tier 2 Provider:
- Great news! The provider is participating in the Evolutions Prime Network.
- Provides the next best member benefit option resulting in a lower copay/cost to members and the plan; however, not as cost-effective as a Tier 1 Provider.
Tier 3 Provider: (Non-contracted providers will not display in the directory.)
- The provider is NOT contracted with Evolutions. You may still seek treatment from this provider as long as the provider allows it.
- The provider will be reimbursed on an amount based on Medicare allowable as per your plan document.
